The All-on-4Ⓡ Treatment Concept

At the Clock Tower Dental practice, in Franklin Square, we call it Smile in a Day but in the professional dental community, it is known as the All-on-4 treatment concept. But, what is the All-on-4 treatment and where did it start?
All-on-4 is a minimally invasive dental implant treatment that was developed and perfected in 1998 by Dr. Paulo Malo. It is a graftless solution, meaning that you do not have to have a bone graft to take place, as you do with other dental implant procedures so that your implant can be placed. Instead, it is a fixed full-arch restoration that uses just 4 tilted implants instead of the usual one implant for each tooth that is being replaced.

The All-on-4 has two straight implants towards the front of the dental prosthesis and two tilted implants in the rear of the prosthesis. The 45º tilt to the implants helps our patients avoid the bone graft procedure, which is so time-consuming for our patients, using the bone that is available.

The All-on-4 has two phases of treatment, a surgical phase where a temporary or provisional bridge is placed, and the second phase, which is the prosthetic phase of treatment. The All-on-4 treatment will give you back a smile that allows you to talk, bite and chew just like you did with your natural teeth.

Patients who are considering the All-on-4 treatment should be in good health. It is contraindicated for those patients who have received significant radiation to their jaw, have a compromised immune system, or are on IV Bisphosphonate therapy.
